Adult scoliosis is less commonly discussed but widely experienced. Some adults have curves that began in adolescence and were never identified. Others develop scoliosis later in life as discs and joints degenerate unevenly. Before discussing curve size in adults, it helps to distinguish between these two pathways. TAlthough the origin differs, both forms involve progressive mechanical stress when left unaddressed.The same Cobb angle ranges used in adolescents are used in adults. What changes is the driver of progression. Growth is no longer a factor, but cumulative load and degeneration can be just as impactful.The chart below shows how adult scoliosis severity is classified using these same ranges. Many adults are surprised to learn how long their curve may have been present before symptoms appeared. Pain often develops after years of uneven loading, not at the moment the curve forms.Moderate and Severe Curves in Adults. As adult curves move into the moderate and severe ranges, symptoms such as pain, stiffness, and nerve irritation become more common. These symptoms are often related to how forces are distributed through the spine rather than the curve alone.The diagram below illustrates how larger curves increase asymmetric stress on discs and joints.At this stage, treatment focuses on improving balance, reducing abnormal loading, and stabilizing the spine. Medications may reduce discomfort, but they do not change the underlying mechanics driving progression.
