Scoliosis and the inner ear balance system (the vestibular system) are linked in research, but no study shows that a spinal curve causes dizziness. A systematic review by Cortés-Pérez and colleagues of 5 studies (154 participants) found asymmetries in the semicircular canals of adolescents with idiopathic scoliosis, correlated with curve location but not severity. In a clinic sample of 159 patients, Vlădăreanu and colleagues found peripheral vestibular dysfunction was associated with larger Cobb angles. Most researchers frame vestibular asymmetry as a possible contributor to the curve, the reverse of what patients usually ask.

I am Dr. Justin M. Dick, DC, at Clear Life Scoliosis and Chiropractic Center in Charlotte, NC. Dizziness is one of the extraspinal complaints I hear from scoliosis patients, and this post explains what the evidence does and does not support.

The Vestibular Findings

Woo and colleagues compared 10 adolescents with idiopathic scoliosis with 10 controls using vestibular stimulation. Every scoliosis patient had more asymmetric one-sided vestibular perception thresholds (0.22 to 1.00 mA against 0.01 to 0.21 mA in controls), while two-sided thresholds did not differ. Cortés-Pérez's review supports structural canal asymmetry. Vlădăreanu also found motion sickness associated with vestibular dysfunction. These are small or single-center samples.

Dizziness, Migraine, and Families

Uneri and colleagues described 7 members of 3 families with idiopathic scoliosis who all had vestibular dysfunction and migraine-type headache, and proposed migraine vestibulopathy as a shared feature. That is a case series. It raises a question; it does not establish a pattern for scoliosis patients in general.

Direction of Cause

The vestibular studies are cross-sectional, so they cannot tell us whether the inner ear asymmetry came first. The leading interpretation in this literature is that vestibular asymmetry may contribute to how the spine develops. I cannot confirm any study showing that treating the curve reduces dizziness.

When Dizziness Needs Medical Workup

New or severe dizziness, dizziness with double vision, slurred speech, facial numbness, weakness, or a sudden severe headache needs emergency evaluation. Dizziness after a car accident or head strike also needs evaluation for concussion and neck injury; our page on neck pain, headaches, and dizziness after an accident covers that. These are standard clinical red flags.

How Dr. Justin M. Dick, DC Approaches Dizziness at Clear Life

At our Charlotte office, dizziness is documented, screened for red flags, and referred to vestibular or neurological specialists when indicated. Balance and righting reflex work is part of some care plans. My labyrinthine righting reflex case report, Reduction of Adolescent Idiopathic Scoliosis Utilizing the Labyrinthine Righting Reflex: A Case Report(Cureus, 2026, PMID 41685001), describes one adolescent whose curve was treated with a protocol that included righting reflex training. It does not test dizziness.

Comparison

Study Design Finding
Cortés-Pérez 2022 Systematic review, 5 studies, 154 people Semicircular canal asymmetry; tied to curve location
Vlădăreanu 2024 Clinic sample, 159 patients Vestibular dysfunction associated with higher Cobb
Woo 2019 10 vs 10 More asymmetric one-sided vestibular thresholds in all scoliosis patients
Uneri 2009 Case series, 7 people Vestibular dysfunction and migraine in 3 families

Research and Evidence

Evidence tiering: Cortés-Pérez is a systematic review of small observational studies; Vlădăreanu and Woo are cross-sectional; Uneri is a case series. None tests whether scoliosis causes dizziness or whether treatment changes it. The red flag list reflects standard clinical practice.

My own published work on reflex-based care is the labyrinthine righting reflex case report cited above, a single-patient report.

Frequently Asked Questions

Can scoliosis cause dizziness?

No study shows that a spinal curve causes dizziness. Vestibular asymmetries are more common in scoliosis, and researchers mainly consider them a possible contributor to the curve.

Is vertigo linked to scoliosis?

Vestibular dysfunction was associated with larger Cobb angles in a sample of 159 patients, but the study cannot show which came first.

Do people with scoliosis have inner ear differences?

A review of 5 studies found semicircular canal asymmetries in adolescents with idiopathic scoliosis, correlated with curve location but not severity.

When is dizziness an emergency?

Dizziness with double vision, slurred speech, facial numbness, weakness, or a sudden severe headache needs emergency evaluation.

Can treating scoliosis help dizziness?

Dr. Justin M. Dick, DC at Clear Life Scoliosis in Charlotte, NC cannot confirm any study showing that treating the curve reduces dizziness. Dizziness is screened and referred when indicated.
