Imaging review identified a structural finding from an old injury that had not been characterized in the prior workups. The point of the case is not that imaging always reframes a diagnosis. It is that when the structural substrate has not been adequately characterized, the differential is incomplete, and patients can spend years cycling through interventions aimed at the wrong problem.
