Definition
ScoliBrace is a custom, 3D over-corrective scoliosis orthosis developed in Australia, characterized by its mirror-image over-correction design philosophy. Unlike traditional braces that aim to contain a curve by applying pressure in the direction of the deformity, ScoliBrace places the entire torso in a mirror-image over-corrected position.
It is fabricated using CADCAM technology from patient-specific data including weight-bearing radiographic measurements, Cobb angle, Lenke classification, and a full 3D body surface scan.
ScoliBrace vs. Boston Brace
Boston brace: Symmetric, applies compression forces along the curve, backed by the BrAIST trial (Weinstein et al., NEJM 2013) -- the only Level 1 evidence for bracing in AIS. Does not address axial rotation.
ScoliBrace differences:
- Over-correction strategy -- places the spine to the opposite side of the deformity using spinal coupling mechanics, addressing both coronal curve and axial rotation.
- 3D customization -- fabricated from the individual patient's radiographic and scan data.
- Active use philosophy -- designed to be used alongside an active rehabilitation program, not as a standalone passive intervention.
Design and Fabrication Process
Fabrication requires: weight-bearing coronal X-ray with documented Cobb angles, lateral X-ray for sagittal profile, Lenke classification with lumbar modifier, full-body 3D scan or detailed body measurements, and clinical photographs.
Wearing Protocol
Standard wearing time: 8-16 hours per day based on age, Cobb angle, skeletal maturity (Risser grade), and curve progression history. First radiographic re-evaluation typically scheduled at 3-6 months. Used alongside active rehabilitation -- never as a brace-only approach.
