No trial has tested mattresses in people with scoliosis. The best mattress evidence comes from chronic low back pain. Kovacs and colleagues randomized 313 adults to a firm or medium-firm mattress in a double-blind trial; at 90 days, the medium-firm group had better odds of improvement in pain while lying in bed (odds ratio 2.36) and in disability (odds ratio 2.10). The belief that a very firm mattress is best for the back was not supported. Sleep quality itself matters: in 1,246 patients with acute back pain, each one-point drop in sleep quality was followed by about a two-point rise in pain the next day.
I am Dr. Justin M. Dick, DC, at Clear Life Scoliosis and Chiropractic Center in Charlotte, NC. Mattress questions come up at almost every scoliosis visit. This post gives the evidence that exists and is clear about what does not.
The Mattress Trial
Kovacs and colleagues enrolled 313 adults with chronic non-specific low back pain who had pain in bed and on rising. Mattresses were rated on a European firmness scale from 1.0 (firmest) to 10.0 (softest); the firm mattress was 2.3 and the medium-firm 5.6. Participants and assessors were blinded. At 90 days, the medium-firm group had better outcomes for pain in bed (odds ratio 2.36), pain on rising (1.93, with a confidence interval that crossed 1), and disability (2.10). Patients with leg pain were excluded, and none were selected for scoliosis.
Sleep and Pain
Alsaadi and colleagues found poor sleep was followed by higher next-day pain in 1,246 patients with recent back pain, independent of depression, and in a separate group of 80 found the relationship ran both ways. A mattress that lets a person sleep is doing part of its job regardless of its firmness rating.
What Is Different With Scoliosis
This section is clinical reasoning, not tested evidence. A curve changes how the trunk contacts the mattress. Side sleepers with a thoracic curve and rib prominence may need enough give at the shoulder and ribs to avoid pressure points. Adults with lumbar curves and stenosis often prefer side lying with knees bent. A mattress cannot straighten a curve, and I would be skeptical of any product claiming it can. My post on sleeping with scoliosis covers positions and pillows.
Practical Guidance
Based on the Kovacs trial and clinical experience: start with a medium-firm mattress, use the store or manufacturer trial period to test it for several weeks, and judge by morning stiffness and pain on rising. A mattress topper is a lower-cost way to adjust firmness. For teens in a night brace, comfort in the brace matters more than the mattress; my post on nighttime bracing covers that.
How Dr. Justin M. Dick, DC Addresses Sleep at Clear Life
At our Charlotte office, sleep position, mattress, and pain on waking are part of the history for every scoliosis and back pain patient. Persistent pain at night despite these changes is a reason for a fresh exam, not just a new mattress.
Comparison
| Question | Finding | Source |
|---|---|---|
| Firm vs medium-firm mattress for back pain | Medium-firm better for pain in bed (OR 2.36) and disability (OR 2.10) | Kovacs 2003, 313 adults, RCT |
| Does poor sleep raise pain? | About 2 points more pain per 1-point drop in sleep quality | Alsaadi 2014, 1,246 patients |
| Mattress trials in scoliosis | None found | I cannot confirm any |
Research and Evidence
Evidence tiering: Kovacs is a double-blind randomized trial in chronic back pain without leg pain; Alsaadi is a large cohort. Applying either to scoliosis is extrapolation, and scoliosis-specific mattress guidance here is clinical reasoning.
My own published work concerns structural care, not sleep surfaces. The 13-month follow-up case, Reduction of Adolescent Idiopathic Scoliosis: A 13-Month Follow-Up (Cureus, 2025, PMID 40062184), describes one adolescent treated without surgery.

Frequently Asked Questions
What is the best mattress for scoliosis?
No trial has tested mattresses in scoliosis. In back pain, a randomized trial of 313 adults found medium-firm mattresses outperformed firm ones for pain in bed and disability.
Is a firm mattress better for back pain?
Not in the best trial. Medium-firm mattresses gave better outcomes than firm mattresses after 90 days.
Can a mattress fix scoliosis?
No. A mattress cannot straighten a curve. Dr. Justin M. Dick, DC at Clear Life Scoliosis in Charlotte, NC treats claims that a mattress corrects scoliosis with skepticism.
Does poor sleep make back pain worse?
Yes. In 1,246 patients with acute back pain, worse sleep quality was followed by higher pain the next day, independent of depression.
Should side sleepers with scoliosis choose a softer mattress?
Possibly, enough give at the shoulder and ribs can reduce pressure points. That is clinical reasoning, not trial evidence.
