Bulging or Herniated Disc After a Car Accident in Charlotte: Reading the MRI Against the Exam

An MRI report that says "disc bulge" or "herniation" after a crash raises two questions: is it causing the symptoms, and was it there before. The report rarely answers either. The exam has to.

At Clear Life Scoliosis and Chiropractic Center in Charlotte, NC, I read disc findings against the neurological exam and the timeline, because the image alone cannot carry the conclusion.

Disc Findings Are Common Without Pain

A systematic review of imaging in people with no back pain found that disc degeneration and disc bulges are common and become more common with every decade of age (Brinjikji et al., AJNR 2015, PMID 25430861). A large MRI study of asymptomatic volunteers found the same pattern in the cervical spine (Matsumoto et al., J Bone Joint Surg Br 1998, PMID 9460946). A bulge on an MRI after a crash is therefore not evidence of injury on its own, and it is not evidence against injury either.

What Makes a Disc Finding Clinically Meaningful

Finding Points toward a symptomatic disc Points away
Symptom pattern Arm or leg pain that follows a nerve root distribution Diffuse pain with no radiating pattern
Neurological exam Reflex, strength, or sensory change at the matching level Normal neurological exam
Level match Finding at the level the exam implicates Finding at an unrelated level
Timeline Radicular symptoms that began after the crash Documented identical symptoms before the crash

When I Refer

Progressive weakness, worsening numbness, or any bladder or bowel change goes to emergency or surgical evaluation immediately. Persistent radicular symptoms that do not respond to an appropriate course of conservative care go to pain management or spine surgery consultation, and the referral reason is documented. Disc care at Clear Life is conservative and phased, and I do not present it as a substitute for surgical evaluation when the exam calls for one.

What the Research Supports, and How Strongly

The prevalence data on asymptomatic disc findings come from a large systematic review and a large MRI cohort, which is strong evidence for the narrow claim that disc findings are common without pain. Evidence for any specific conservative disc treatment after trauma is more limited. Dr. Justin Dick's publications are listed on the research page, including a collision case report at the lowest evidence tier: Refractory Lumbar Pain Following MVA in a Geriatric Patient.

Frequently Asked Questions

Can a car accident cause a herniated disc?

A collision can injure a disc, and it can also aggravate a disc that was already degenerated. Because disc bulges and degeneration are common on MRI in people without pain, a disc finding after a crash is interpreted against the symptom pattern, the neurological exam, and the timeline rather than read on its own.

Does a disc bulge on my MRI mean the accident caused it?

Not by itself. Imaging studies of people without back or neck pain show disc bulges are common and increase with age. Whether a bulge is related to a crash depends on whether new symptoms and neurological findings match that level and began after the collision.

When is a disc injury an emergency?

Progressive arm or leg weakness, rapidly worsening numbness, numbness in the groin or saddle area, or any change in bladder or bowel control requires emergency evaluation.
