Low Back Pain After a Rear-End Collision in Charlotte: Red Flags, Imaging, and Care
Whiplash gets the attention, but the lumbar spine absorbs load in a rear-end collision too, especially when the occupant is seated with the pelvis fixed by the lap belt. Low back pain after a crash is common in my Charlotte practice and is often under-documented because the neck dominated the first visit.
This post covers what I screen for, when imaging is indicated, and how lumbar complaints get recorded alongside the neck.
Red Flags That Need Emergency Evaluation
Numbness in the groin or inner thighs, new bladder or bowel dysfunction, progressive leg weakness, fever, a history of cancer, significant osteoporosis, or severe pain after a high-energy crash in an older patient all point to an emergency or urgent imaging workup, not a chiropractic office. I screen for these at every collision intake.
Imaging Decisions for the Low Back
In the absence of red flags, most acute low back pain does not need immediate advanced imaging. Lumbar MRI findings such as disc bulges and degeneration are common in people without pain and increase with age (Brinjikji et al., AJNR 2015, PMID 25430861), so an MRI ordered without a clinical question produces findings that are hard to interpret. Radiographs are used when trauma severity, age, or exam findings raise fracture or structural concern. MRI is reserved for persistent radicular signs or failure of appropriate care.
How Lumbar Complaints Are Charted
| Recorded at intake | Recorded at each re-exam |
|---|---|
| Onset relative to the crash, separate from neck onset | Pain location and leg symptoms compared to intake |
| Seated position, lap belt use, direction of impact | Lumbar range of motion and provocative testing |
| Lower extremity reflexes, strength, and sensation | Neurological findings by level |
| Prior low back history | Function: sitting tolerance, driving, work |
What the Research Supports, and How Strongly
The asymptomatic imaging data are strong. Evidence for specific conservative treatments of post-collision low back pain is more limited and mixed. Dr. Justin Dick published a case report on refractory lumbar pain after a motor vehicle accident in a geriatric patient. It is a single case and the lowest tier of clinical evidence, and it does not establish that the result generalizes: Refractory Lumbar Pain Following MVA in a Geriatric Patient.
Frequently Asked Questions
Can a rear-end collision cause low back pain?
Yes. The lumbar spine absorbs load in a rear-end collision, and low back pain is a common complaint after crashes. Lumbar symptoms should be documented with their own onset date, separate from neck symptoms.
When is low back pain after a car accident an emergency?
Numbness in the groin or inner thighs, new bladder or bowel dysfunction, progressive leg weakness, fever, a history of cancer, or severe pain after a high-energy crash in an older patient requires emergency or urgent evaluation.
Do I need an MRI for low back pain after a car accident?
Usually not right away. Without red flags, most acute low back pain does not need immediate MRI, and disc findings are common in people without pain. MRI is indicated for persistent leg symptoms with neurological signs or failure of appropriate care.
Has Dr. Justin Dick published research on back pain after car accidents?
Yes. Dr. Justin Dick of Clear Life Scoliosis and Chiropractic Center in Charlotte, NC published a peer-reviewed case report on refractory lumbar pain following a motor vehicle accident in a geriatric patient. As a single case report, it is the lowest tier of clinical evidence.
