After posterior spinal fusion for adolescent idiopathic scoliosis, most published programs discharge patients in about 3 to 5 days, most teens return to full-day school at about 6 to 7 weeks, and most return to sports between 8 and 12 months. In a 2026 prospective study of 118 patients at Scottish Rite for Children, families expected 31.4 days out of school and the actual mean was 46.8 days, about two weeks longer than planned.
I am Dr. Justin Dick at Clear Life Scoliosis and Chiropractic Center in Charlotte, NC. I do not perform surgery. I see families before surgery deciding on options and patients after fusion with pain above or below the hardware. This post gives the recovery numbers families ask me about, with the sources.
The Hospital Stay
Length of stay depends on the hospital's protocol. Sanders and colleagues at Children's Hospital Los Angeles compared 194 patients on a traditional pathway with 90 on an accelerated discharge pathway. Average stay fell from 5.0 to 3.7 days, with no significant change in wound complications (3.6 and 3.3 percent) or readmissions (1.5 and 4.4 percent), and a small rise in reported pain on days 2 to 4. Ahdoot and colleagues reported 3.3 days on a rapid recovery pathway against 4.4 days on a conventional one, with earlier physical therapy clearance and no difference in total narcotic use. Ask the surgical team which pathway they use, because it sets expectations for the first week.
Return to School
Boeckmann and colleagues followed 118 patients prospectively. The mean return to full-day, in-person school was 6.7 weeks. Families underestimated it by roughly two weeks. Fusion length, sex, neighborhood deprivation index, and race did not predict the timing. Khaleeq and colleagues, in a UK series of 66 respondents with an average of about 10 fused levels, found 89.3 percent returned to school between 1 and 3 months. Planning for six to seven weeks, with schoolwork arranged for home in the meantime, matches the data better than planning for four.
Return to Sports
Nassar and colleagues pooled 11 studies with 722 patients and found most resumed sports between 8 and 12 months. Delays were linked with older age, larger curves, longer and lower fusions, fear of injury, anxiety, and deconditioning. Pepke and colleagues found an average of 8 months in 113 patients, a shift from contact to non-contact sports after surgery, and that only 33 returned to exactly the same activities. Khaleeq reported 63.6 percent returned to contact sports, mostly at 7 to 12 months. Surgeon advice varies: in a survey of 61 spine surgeons by Cabrera and colleagues, about one in five allowed contact or collision sports only after 12 months or more. No validated return-to-sport guideline exists.
What Recovery Does Not Restore
A fused segment does not move again. Aaro and Öhlén, in an older Harrington-era series of 96 patients, found that the lower the fusion extended into the lumbar spine, the less lumbar flexion and lordosis remained and the more stiffness and pain patients reported. Modern instrumentation differs from Harrington rods, but the principle that fused levels do not bend still holds. My post on bending after scoliosis surgery covers mobility in detail, and scoliosis rods long term covers hardware.
How Dr. Justin Dick Supports Families Before and After Fusion
At our Charlotte office, families considering surgery get a radiographic review and a plain account of non-surgical options that fit the curve, and patients after fusion get care for the unfused segments, which carry more motion. Our page on scoliosis care after spinal fusion describes that work. Surgical recovery questions belong with the surgeon's team.
Comparison
| Milestone | Published figure | Source |
|---|---|---|
| Hospital stay, accelerated pathway | 3.7 days (vs 5.0 traditional) | Sanders 2017, 284 patients |
| Hospital stay, rapid recovery pathway | 3.3 days (vs 4.4 conventional) | Ahdoot 2021, 44 patients |
| Return to full-day school | Mean 46.8 days (6.7 weeks); families expected 31.4 | Boeckmann 2026, 118 patients |
| Return to school, 1 to 3 months | 89.3% | Khaleeq 2025, 66 respondents |
| Return to sports | Generally 8 to 12 months | Nassar 2026, 11 studies, 722 patients |
| Same sport as before surgery | 33 of 113 patients | Pepke 2023 |
Research and Evidence
Evidence tiering: Nassar is a systematic review of observational studies, Boeckmann a prospective cohort, Sanders and Ahdoot retrospective cohorts, Pepke and Khaleeq retrospective surveys, and Cabrera a surgeon opinion survey. No randomized trial sets a return-to-sport date. Single-center discharge figures reflect each hospital's protocol and may not transfer to another hospital.
My own published work concerns non-surgical care. The 13-month follow-up case, Reduction of Adolescent Idiopathic Scoliosis: A 13-Month Follow-Up (Cureus, 2025, PMID 40062184), reports one adolescent managed without surgery. It is a single case and does not compare against fusion.

Frequently Asked Questions
How long is the hospital stay after scoliosis surgery?
In published pediatric programs, about 3 to 5 days. An accelerated pathway at one hospital cut average stay from 5.0 to 3.7 days without a significant change in wound complications.
When can my teen go back to school after spinal fusion?
A 2026 prospective study of 118 patients found a mean of 46.8 days, about 6.7 weeks, to full-day in-person school. Families had expected about 31 days.
When can you play sports after scoliosis surgery?
Most patients return between 8 and 12 months, according to a 2026 systematic review of 722 patients. There is no validated guideline, and surgeons' advice varies.
Can you go back to contact sports after spinal fusion?
Many do. One UK series reported 63.6 percent returned to contact sports, mostly at 7 to 12 months, but other studies found a shift toward non-contact sports after surgery.
Will my back bend normally after fusion?
Fused segments do not move. The lower a fusion extends into the lumbar spine, the more flexion is lost. Dr. Justin Dick at Clear Life Scoliosis in Charlotte, NC sees patients after fusion for the unfused segments.
