Stretching can help chronic low back pain when it is done as a structured class, but on average it ranks below other forms of exercise. In a randomized trial of 228 adults, a 12-week stretching class performed about as well as a 12-week yoga class, and yoga beat a self-care book. In a network meta-analysis of 89 trials, though, stretching alone did not differ from no treatment for pain or function, while Pilates, stabilization, and resistance training ranked highest. Exercise as a category has the strongest support: a Cochrane review of 249 trials found it reduced chronic low back pain by about 15 points on a 100-point scale.
I am Dr. Justin Dick at Clear Life Scoliosis and Chiropractic Center in Charlotte, NC. Patients ask for "a few stretches." This post explains why I usually give them something closer to a training program.
Exercise as a Whole
Hayden and colleagues' 2021 Cochrane review included 249 trials in chronic non-specific low back pain. Compared with no treatment, usual care, or placebo, exercise reduced pain by 15.2 points on a 100-point scale, which met their threshold for a clinically important difference. The function improvement, 6.8 points, did not. Exercise was not different from manual therapy for pain. Most adverse effects were minor, such as muscle soreness.
Stretching Compared With Other Exercise
Owen and colleagues' network meta-analysis of 89 trials and 5,578 patients ranked exercise types against true control. Pilates ranked highest for pain, resistance and stabilization training for function, and resistance and aerobic training for mental health. Stretching and McKenzie exercise were not different from true control for pain or function. The authors rated the evidence quality low.
The Stretching Class Trial
Sherman and colleagues randomized 228 adults with chronic low back pain to 12 weekly yoga classes, 12 weekly conventional stretching classes, or a self-care book. At 12 weeks, yoga was better than the book for function and symptoms, and the benefit for function lasted to 26 weeks. Yoga was not better than the stretching classes at any time point. Supervised, progressive stretching sessions are a different intervention from a few stretches done occasionally at home, and that difference may explain why Owen's pooled result for stretching was weaker.
Acute Back Pain Is Different
These trials studied chronic pain lasting more than 12 weeks. For a new episode, staying active matters more than a specific stretch. Steffens and colleagues found that heavy loads and distraction during tasks were strong triggers of new episodes, and da Silva and colleagues found 69 percent of people had a recurrence within a year, more often with long sitting and awkward postures. Those findings point toward load management and conditioning.
When Stretching Is Not the Answer
Leg pain below the knee, numbness, or weakness suggests nerve involvement, and stretches that tension the nerve can aggravate it. My post on pinched nerve vs herniated disc covers that. Back pain with fever, weight loss, a cancer history, or bladder changes needs medical evaluation, not a stretching program.
How Dr. Justin Dick Builds Back Programs at Clear Life
At our Charlotte office, a home program follows an exam that checks movement, strength, and alignment. Programs favor progressive strength and control work, with stretching where a specific restriction is found, and are combined with spinal care when indicated. My post on ergonomics at work covers the sitting side.
Comparison
| Approach | Effect in chronic low back pain | Source |
|---|---|---|
| Exercise overall vs no treatment or usual care | Pain 15.2/100 lower | Hayden 2021, 249 trials |
| Stretching alone | Not different from control | Owen 2020, 89 trials |
| Pilates | Ranked highest for pain | Owen 2020 |
| Stretching classes vs yoga classes | No significant difference | Sherman 2011, 228 adults |
| Yoga vs self-care book | Yoga better for function and symptoms | Sherman 2011 |
Research and Evidence
Evidence tiering: Hayden is a Cochrane review, Owen a network meta-analysis with low-quality evidence, Sherman a randomized trial, Steffens a case-crossover study, and da Silva a cohort. The results apply to chronic non-specific low back pain and should not be applied directly to sciatica or specific spinal conditions.
My own published work concerns structural spinal care. The refractory lumbar pain case, Refractory Lumbar Pain Following Motor Vehicle Collision in a Geriatric Patient With Prior Lumbar Surgery (Cureus, 2026, PMID 41694169), describes one patient.

Frequently Asked Questions
Do stretches help lower back pain?
Supervised stretching classes performed about as well as yoga in a trial of 228 adults, but a network meta-analysis of 89 trials found stretching alone was not different from control.
What is the best exercise for chronic low back pain?
In a network meta-analysis, Pilates ranked highest for pain, and resistance and stabilization training for function. Exercise overall reduced pain about 15 points on a 100-point scale.
Is yoga better than stretching for back pain?
Not in the Sherman trial. Yoga was better than a self-care book but not better than conventional stretching classes at any time point.
Should I stretch with a new back injury?
For acute pain, staying active and managing loads matters more than specific stretches. Dr. Justin Dick at Clear Life Scoliosis in Charlotte, NC evaluates new back pain before prescribing exercise.
When should I not stretch my back?
With leg pain below the knee, numbness, or weakness, or with fever, weight loss, or bladder changes. Those need evaluation first.
